Food Order Form for Home Football Games

Marching Band:  Please print out this form to place your dinner order for home football games to ensure that you get the meal you want.

It is preferable (not a requirement) that you place your order and pay for all 5 home games prior to the start of the season.  Please place your form and a check/cash in an envelop and provide it to Mr. Martin or Mrs. Paul.  Please make checks payable to LHS Band Boosters Inc.

Deadline for each week is the Thursday before the football game.
